Parental Permission

I agree to the below Terms and Conditions *

1. I give permission for my child/children to attend and fully participate in Soulies HSC Study Hall 2024

2. Whilst every precaution will be taken to ensure the good welfare and protection of my child, St Faiths Anglican Church, their staff, leaders and others acting on their behalf are hereby released from any liability in the event of accident or misfortune, damage or loss that may occur to students or their property

3. In the case of a medical emergency, I hereby give permission to the doctor chosen by the St Faith's staff to secure proper treatment for my child

4. During the course of the Study Hall, I am aware that my child may be captured on photograph or video that may be used by St Faiths for promotional or historical purposes.
